Letter from Employer, Payslips or Group Certificates, Letter from Centrelink, Etc.) • Team List – Players Names and Date of Birth/Ages • Enclosed with your application you should have: 1. Application Form 2. Confirmation of Aboriginality 3. Income Details 4. Acquittal Form. If you do not have any of these forms please contact this office on the above number.
